Day of Caring - Volunteer

Volunteer Application

Complete this form to register for Day of Caring. Volunteers are the "caring" of Day of Caring. We welcome old and new friends, family, neighbors, co-workers, churches, civic groups and anyone else who is ready to invest time to volunteer for the annual Day of Caring!

If you are not employed, mark NA or type in RETIRED.
Get your T-shirt! The earlier you register the higher likelihood you will get the right size.
If you are comfortable with specific types of work, please select from the list your skills. To choose more than one skill, hold the shift key as you click on the skills. If you don't see your skill, let your project team lead know what it is. Hold the control button to choose more than one option.
Do you have equipment or supplies you can provide to offset the cost of a project? If so, please keep in mind that you are responsible for the equipment to include marking each with your identifier to ensure equipment is returned to the proper owner. To choose more than one item, hold the control key as you click on the items.
If you have a preference for a particular day to volunteer please make a selection. To choose both, hold the control key, then click on both dates. THESE DATES ARE NOT FINALIZED YET. Serious inclement weather will make most projects impossible to complete. A rain date is tentatively set for November 9, 2019.
If you have a preference of where you volunteer please make a selection. The committee will do its best to schedule you for your choice, but cannot guarantee it due to number of projects and volunteers.
Please provide the agency name or home address if residential. If not known, input "PROJECT NAME UNKNOWN"

Volunteer Waiver

In consideration of being permitted to participate in the Day of Caring, Volunteer agrees to release and forever discharge and hold harmless the United Way, its successors and assigns, collectively or individually, from any and all liability, claims and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from Volunteer’s work as part of the Day of Caring. This includes all work being undertaken at any project site including but not limited to, United Way member agencies, other non-profit organizations, residents who are having work undertaken at their private homes, and any other party having a legal interest in the property on which Day of Caring projects/events take place. Volunteer also acknowledges that United Way does not assume any responsibility for or obligation to provide financial assistance or any other assistance, including but not limited to medical, health or disability insurance in the event of injury or illness to the Volunteer arising from the Day of Caring.
2. Medical Treatment. Except as otherwise agreed to by United Way in writing, Volunteer does hereby release and forever discharge United Way from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, medical care or service rendered in connection with the Volunteer’s work relating to the Day of Caring.
3. Assumption of Risk. The Volunteer acknowledges that the work undertaken as part of the Day of Caring may include activities that may be hazardous to the Volunteer. Volunteer hereby expressly and specifically assumes the risk of injury or harm in these activities and releases United Way from any and all liability for injury, illness, death or property damage resulting from the Volunteer’s activities relating to the Day of Caring
4. Insurance. The Volunteer acknowledges that United Way does not carry or maintain health, medical or disability insurance coverage for any volunteer. EACH VOLUNTEER IS ENCOURAGED TO OBTAIN HIS OR HER OWN MEDICAL AND/OR HEALTH INSURANCE COVERAGE.
5. Photographic Release. Volunteer agrees to and permits United Way to take photographic images and video and audio recordings of him/her during his/her work relating to the Day of Caring. Volunteer also grants and conveys to United Way all rights, titles and interests in said photographic images and video and audio recordings, including but not limited to, any royalties, proceeds or other benefits derived from such photographs or recordings. Volunteer agrees that in the event that any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release which shall continue to be in full force and effect.
7. Effective Date. This Release shall be effective immediately upon execution and shall cover any and all work Volunteer performs before, during and after November 2018, in relation to their assigned project and the Day of Caring.
I hereby waive all claims for damage or loss to my person or property which may be caused by any act or failure to act of the United Way of Greater Baytown Area & Chambers County, its officers, agents or employees or any of the United Way affiliated agencies, its officers, agent or employees. I assume the risk of all dangerous conditions in and about property where I am doing volunteer service and waive any and all specific notice of the existence of such condition.
You must agree to the entire Volunteer Release and Waiver Liability in order to proceed. This serves as your agreement. You may also be required to complete/sign a physical waiver.
As a volunteer, you are required to work safely and within your abilities. At times, a lead volunteer may ask a volunteer to change to a project that is better suited to their skills or if that volunteer is not wearing the correct protective clothing. It is highly recommended you consider the following: Wear appropriate safety clothing and footwear for the assigned project. Bring water and snacks; provide your own lunch unless the project team lead tells you it will be provided. If the project is outdoors, wear sunscreen and a hat. Notify the project lead of any allergies or medical concerns. Complete the volunteer waiver and provide to United Way and/or the project lead. You will not be able to participate without this document. Please select yes if you understand and will adhere to these recommendations.
